Dear UWF Student-Athletes,

Physicals will be offered at the beginning of the semester prior to competition. Upon enrollment to UWF, you will receive an e-mail from your athletic trainer with instructions to fill out your new athlete paperwork. We ask that you prepare by compiling medical documentation (ie: surgical notes, doctor notes, X-Ray reports, and MRI results, etc.)  from any significant injury within the most recent five years. All documentation must be completed and submitted to the UWF Sports Medicine Staff prior to participation. If you need any assistance, please contact the athletic training room at (850) 474-2516.
